Position Summary:
The Clinical Coordinator is responsible for supporting and leading the dedicated nursing staff on multiple floors in the memory care unit. The Clinical Coordinator ensures staff assignments provide care that enhances residents' potential; collaborates with medical providers; assists with staff development; and oversees clinical operations leading to positive outcomes.
The schedule for this position is primarily Monday-Friday, day shift w/ occasional weekends. No on-call or scheduling responsibilities.
Pay is commensurate with years of RN and leadership experience. Generous extra pay for hours worked past 3:00 pm and on weekends.
Responsibilities:
Responsible for coordination of clinical services for each resident
Oversees the development of individualized care plans
Generates nursing staff assignments, assuring follow through including complete and accurate written and verbal communication to support the care needs of each resident
Establishes a compassionate environment by providing emotional, psychological, social and spiritual support to residents, friends, and families
Assists floor nurses with bedside care when needed, performs periodic rounds on floors and assists with Nursing Department audits
Ensures that all staff embraces and lives the St. John’s Brand Characteristics: Friendly, Respectful, Responsive, Compassionate, Innovative, and Fun as it applies to their duties as a St. John’s employee
Qualifications / Education:
Current RN license in NYS required + a minimum of one (1) year of long term care or acute care experience required
Minimum of one (1) year formal leadership experience in health care required
Current CPR certified required
Excellent communication skills
Willingness to support and lead others to living St. John’s Brand Characteristics
Compliance with NYS Department of Health immunization requirements.
Physical Requirements:
Sits, stands, walks most of the day. Frequent moving and lifting of residents. Moderate to heavy lifting (up to 50 lbs).
Exposure to Conditions:
May be exposed to infection, odors, psycho-behavioral situations.
