Full-Time, Monday-Friday, Flexible Schedule (based on participant needs)
No weekends or holidays!

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role
We are looking for a proactive employee to provide services to children and adults with a range of developmental, physical and/or health related disabilities. We are looking to hire a Physical Therapist, a Speech Language Pathologist, and a Licensed Clinical Social Worker!
Responsibilities include:
Evaluations and revaluations for individuals on caseload
Development of Plan of Care (POC)
Complete treatment plans, evaluations, billing forms and discharge summaries in a timely manner
Maintain appropriate contact with assigned participants and their families, guardians and care managers
Education & Experience
For PT: Doctorate and current NYS License in PT required. 1 year experience with evaluation of PT services to individuals with disabilities, preferred.
For SLP: Master’s and current NYS License in SLP required. 1 year experience with evaluation and provision of speech therapy services to individuals with disabilities, preferred.
For LCSW: Current NYS LCSW license required. 1 year experience working with children and adults with developmental disabilities and/or a mental health diagnosis, preferred.
Knowledge, Skills & Abilities
Excellent written and verbal communication skills, required. Basic word processing and email skills, required. Position involves access to confidential information that must be respected.
Conditions of Employment
Must possess a valid driver’s license and transportation necessary to meet program needs. Must be able to lift 75 pounds. Must be willing to have a flexible schedule based on therapy needs of assigned caseload.
