Assistant Program Manager – Day Habilitation
Reports to: Networks Day Habilitation Program Manager, DD/ABI
Location: Onondaga County
Full-Time: M/W/F 8:30a-4:30p, T/TH 12:30p-8:30p

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.
About the Role
We are looking for a proactive employee to provide person centered services and support to individuals to achieve independence while maintaining their health and safety.
Responsibilities include:
Assisting Program Managers with day to day operations of Community Habilitation services
Implementing participants goals as written in Action plan
Assisting and supporting participants with ADL (activities of daily living) skills including bathing, toileting, dressing, and mobility as needed
Monitoring and maintaining participants safety while in their home, the community, and during transportation
Education & Experience
High school diploma/GED plus 1 year experience working with individuals with disabilities, required.
Knowledge, Skills & Abilities
Basic word processing and e-mail skills required.
Conditions of Employment
Must have a valid driver’s license and transportation necessary to meet program needs. Must be able to lift 75 pounds.
