Position Overview:

We are seeking a dedicated and compassionate Service Coordinator to join our team at East House. As a Service Coordinator, you will play a vital role in assisting clients in their recovery from mental illness or chemical dependency. Your primary responsibility will be to help clients identify, access, and maintain essential services, including treatment, housing, rehabilitation, and community support, to enable them to lead independent lives and improve their overall quality of life.

Key Responsibilities:

Client-Centered Service Planning:

  • Collaborate with clients to develop individualized Service Plans aligned with their goals and objectives.

  • Manage a caseload to coordinate a wide range of client services within the community, advocating for mental health, chemical dependency, financial, legal, career, and recreational services.

  • Interact independently with clients in their homes and community settings, demonstrating knowledge of mental illness and sound judgment.

  • Participate in case reviews to ensure service quality and client stability.

  • Maintain a strong knowledge base of best/evidence-based practices and community resources, actively pursuing additional training and education as needed.

  • Identify service gaps and collaborate with the Program Director to adapt services to changing demographics and client needs.

  • Implement crisis management processes and procedures when required.

  • Communicate significant client information to supervisors and community service providers.

  • Meet units of service standards.

  • Participate in annual program objective planning.

  • Link clients to appropriate social support networks.

Effective Housing Support Services:

  • Collaborate with clients to locate and maintain suitable, affordable housing using available resources.

  • Advocate and support positive client-landlord relationships and teach communication skills with neighbors.

  • Develop and maintain positive relationships with landlords, neighbors, community groups, referral sources, and service agencies.

  • Facilitate client access to housing subsidies, utility assistance programs, food pantries, and other community organizations.

  • Minimize premature lease terminations and address any client complaints regarding housing assistance.

Clients’ Benefits and Income Management:

  • Assist clients with federal and county benefit applications and recertifications.

  • Advocate for appropriate funding sources.

  • Teach clients budgeting skills.

  • Ensure 100% of clients receive assistance with maximizing entitlements and gaining self-sufficiency skills.

Coordinated Services to Family Members:

  • Facilitate access to mental health, educational, and social services for dependent children of clients.

  • Promote the self-sufficiency of the household network using family development practices.

  • Assist clients in researching, identifying, and accessing resources for dependent children's special needs.

  • Coach parents to actively participate in school planning, health, and recreation programs for dependent children.

  • Provide education and support to family and friends of clients.

  • Maintain documentation in compliance with funder requirements and East House policies and procedures.

Working Conditions:

  • Traditional office or household environment.

  • Occasional pressure due to time demands.

  • Occasional travel to other East House facilities.

  • Availability to work weekends and some evenings.

  • Frequent local travel and client transportation.

    Physical/Mental Demands:

  • Regular office tasks such as sitting, handling paperwork, computer use, and communication.

  • Occasional lifting (up to 30 lbs.), walking, standing, and reaching.

  • Finger dexterity for telephone and computer operation.

  • Adequate hearing and vision for normal conversations and reading documents.

  • Emotional control in stressful situations.

  • Ability to travel locally and access client homes, which may not be handicap accessible.

Requirements

Service Coordinator Job Levels:

Service Coordinator I:

  • High School Diploma + 4-6 years of experience or equivalent.

  • Autonomy: Performs day-to-day duties independently but may seek additional support or feedback when issues arise. Handles some complex issues independently but seeks outside support when needed.

  • Interpersonal Interaction: Works well with clients and employees, assisting others as necessary to accomplish agency goals, and displays appropriate behaviors.

  • Agency Involvement: Involved in a limited number of agency committees, focus groups, teams, etc.

Senior Service Coordinator:

  • High School Diploma + 6-8 years of experience or equivalent.

  • Autonomy: Performs all duties regularly with minimal supervision. Comfortably handles most complex issues with little need for outside consultation.

  • Interpersonal Interaction: Same as Service Coordinator I, plus actively assists other employees in accomplishing agency goals and routinely acts as a leader or mentor for other staff.

  • Agency Involvement: Regularly involved in agency committees, focus groups, teams, etc.

Other Skills/Knowledge/Experience:

  • Demonstrated knowledge of counseling culturally diverse individuals with psychiatric and/or chemical dependency problems.

  • Strong counseling, crisis assessment, and intervention skills within a psychiatric rehabilitation framework.

  • Effective communication skills and the ability to work collaboratively with diverse clients, staff, and community representatives.

  • Ability to assess, monitor, and evaluate program needs and progress.

  • Basic computer literacy, including internet use, email, word processing, and database software.

Salary Description

$17.65-19.00
