Jordan Health is now seeking aPerinatal Case Manager/LPNwho, under the guidance of the Practice Manager, is primarily responsible for providing overall wellbeing of pre- and post-natal patients at Woodward through education and counseling.

Requirements

ThePerinatal Case Manager/LPNwill ensure to:

· Encourage and facilitate scheduling and keeping prenatal appointments, postpartum checks and beginning of care of the infants. (Follow up positive pregnancy test, broken appointments, transfers, procedures, etc.).

· Conduct Hospital and/or home visits as needed.

· Act as liaison between OB provider, Health Center Provider and patient. Promotes wellbeing of the patient through education.

· Take triage calls for OB/GYN patients.

· Scheduling initial prenatal intakes and OB Ultrasounds as ordered by Provider.

Education And Experience Required:

· Accredited LPN Program or Human Service Degree.

Licenses And Certifications Required:

· Current NYS LPN License without any negative actions or Human Service Degree.

· Certification to administer medications.

· Current provider BLS/CPR through the American Heart Association.

· Infection Control Certificate.

Special Skills, Knowledge Required:

· OB related background.

· Visiting nurse/community outreach experience.

· Facility in a second language would be helpful.
