Pay Range: $88,000 - $89,700 / annually (Pay information will vary depending upon relevant experience for the position.)

Full-Time Day - Monday - Friday

Essential Job Duties

  • Be familiar with Church Home mission statement and the nursing department philosophy. Must be able to discuss their concepts in meetings and whenever an opportunity occurs in the practice area.

  • Be familiar with and able to interpret and enforce all policies and regulations. Consistently practice compliance with established policies and procedures.

  • Maintain awareness of government regulatory process.

  • Use the ANA standards of gerontological nursing practice as framework for professional responsibilities.

  • Support nursing colleagues in their professional development toward common goals.

  • Attend and participate in nursing department leadership meetings.

  • Attend in-service programs, in and outside the facility consistent with nursing practice.

  • Develop and foster staff morale and teamwork and be a positive role model.

  • Be responsible for the implementation of the nursing goals on the unit.

  • Assist with staff orientation on your unit in collaboration with the staff education coordinator. Assess the skills and educational needs of staff. Communicate with the staff education coordinator. Facilitate staff to attend workshops, seminars, continuing education programs.

  • Provide opportunities for staff to participate in actualizing goals and plans of the nursing department.

  • Complete and execute performance evaluations when due.

  • Utilize the progressive discipline process when staff counseling is warranted.

  • Hold monthly and as needed staff meetings. Submit a report of all staff meetings, along with an attendance list to the Staff Education Coordinator.

  • Assess and identify residents’ needs and communicate this to other health care professionals (clinical associates, nursing administration, nurse practitioners, physicians, dietitians, therapists, social work) on a regular and timely basis.

  • Attend or ensure representation at morning report daily.

  • Initiate and/or participate in quality assurance monitoring and evaluation of resident care.

  • Conduct walking rounds on unit to assess the environment for safety issues.

  • Create an environment of respect for the residents. Assist and guide staff to maintain privacy, resident rights and a quiet, peaceful environment. Encourage resolutions of issues.

  • Assign nursing staff members resident care activities and other duties.

  • Responsible for the supervision of all nursing staff on unit.

  • Make recommendations to the Director of Nursing related to the nursing practice and resident care through regular meetings/written proposals.

  • Participate in the interviewing and hiring and termination process of applicants for nursing positions.

  • Develop and implement care plans per regulations.

  • Notify responsible party of change in resident condition in collaboration with the social worker.

  • Familiar with requirements specific to MDS+. Develop and complete MDS nursing portion. Complete and review CAA’s as required.

  • Transcribe and implement physician orders per policy/procedures.

  • Follow established documentation procedures and enforce correct documentation procedures with staff.

  • Assign nursing responsibilities on the unit.

  • Maintain contact and ensure compliance with the skin integrity program.

  • Ensure care plans and care cards are updated as resident’s condition and/or orders change.

  • Review of EMR to ensure they are accurate.

  • Review pharmacy consultant reports with the medical team.

  • Be familiar with job descriptions for all staff on the nursing unit and ensure compliance and expectations are met.

  • Be familiar with unit’s budget; manage it and maintain within parameters with Director of Nursing guidance.

  • Identify areas where cost control is needed. Discuss observations with director of nursing.

  • Clarify cost control awareness with staff in monthly meetings as indicated. Work with staff in projects to reduce cost while maintaining the high quality of care.

  • Utilize supplies and staff time in a cost effective manner.

  • Respond to emergency situations according to established Church Home disaster plan.

  • On call responsibility on a rotating basis per nursing department needs.

  • Report unsafe condition to security, maintenance as soon as possible.

  • Document and follow through with all incidents/accidents and follow through per policy.

  • Familiar with established emergency procedures and staff incidents.

Requirements

QUALIFICATIONS:

  • Current registered nurse licensure in New York State.

  • Five years’ clinical experience, one in gerontology and one year of management experience preferred.

  • Maintain confidentiality of all information related to the organization, residents, participants, family and staff, that may be encountered, either formally or informally, during the normal course of business. This includes medical and treatment records, financial and human resources information.

WORKING CONDITIONS:The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Being located at the main entrance of the building may expose the employee to drafts and temperature fluctuations.

SUPERVISORY RELATIONSHIP:The work is performed within established guidelines, and requires periodic supervision. The employee exercises some initiative and independent judgment to effectively perform the essential job duties of this position.

RESPONSIBILITY FOR OTHERS:The employee has direct responsibility for others. Will be required to assist in on-going training and coaching of staff.

SAFETY RESPONSIBILITIES:Shall be fully acquainted with all safety policies and procedures of the Church Home. Takes an active role in all fire drills during the shift and/or emergencies for internal and external disasters as monitored by supervisor and incident manager. Must comply with facility in-service requirements.
