University of Rochester Mental Health Intake Cnslr I in Rochester, New York
GENERAL PURPOSE:
This job consists of patient coordination of all new requests for services which can encompass up to 100 requests per week. The job involves extensive tracking via spreadsheets to ensure no patients fall through the cracks. It also involves communication via phone, MyChart, letters and inbasket messages with patients, families, and referring clinicians. The job requires a high level of competency and independent critical thinking.
RESPONSIBILITIES:
Patient Intake and Service Coordination:
Organize and track new patient requests on an excel spread sheet along with printed phone screens and pertinent clinical information and provide to Clinical Lead and Director
Disseminate disposition information from the new patient intake log to all necessary parties.
Attends Intake Team meetings (2 times/week) and weekly supervision
Conducts intake phone screens with potential patient/families. Documents in eRecord patient charts.
Works collaboratively with, and covers for other Intake Team members when they are not available
Schedules new patient visits as needed
Handles patient complaints and inquiries regarding intake issues, including correspondence; assures that patients are given prompt, courteous attention by telephone or by letter
Serves as a focal point for answering questions and solving problems regarding intake issues for difficult or unusual circumstances, assists in problem identification and resolution
Communications Coordination for Intake Team:
Composes, customizes and sends correspondence to families seeking care within our services (e.g., waitlist and referral correspondence; requests for clinical summaries for psychiatric consultation)
Communicates via eRecord and email to clinicians within the multi-site service
Communicates with primary care providers, referring specialty pediatrics providers, and school personnel as needed
Assures that patient and patient families are given prompt, courteous attention by telephone or by letter
Liaison Responsibilities:
Interfaces with our Brighter Days Pediatric Mental Health Urgent Care on patients needing access to crisis services and being transferred from the Urgent Care
Coordinates with Financial Counselors regarding fees and insurance issues
Establishes and regularly maintains cooperative working relationships with community and health care agencies and professional and administrative staff of the Medical Center and University.
Handles inquiries from other UR Medicine clinical services (e.g. Inpatient psychiatry, pediatric partial hospitalization in psychiatry, and pediatric clinics) as well as community providers (e.g. PCPs, school district staff) when the Clinical Intake Coordinator is unavailable.
Interfaces with clinical teams associated with research projects being conducted at the service.

QUALIFICATIONS:
- Bachelor’s degree or an equivalent combination of education and experience preferred.
Required:
Proficient computer experience (with programs such as Excel)
Excellent organizational skills
Excellent oral and written communication skills Excellent attention to details
Excellent internal and external customer service skills (e.g., with families, physicians, mental health clinicians, trainees)

Pay Range
Pay Range: $24.40 - $34.16 Hourly
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
