Honor Community Health Targeted Case Manager in Pontiac, Michigan

Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. Position Description
Reporting to the Project Director for the CCBHC the Targeted Case Manager performs activities within the Certified Community Behavioral Health Clinic that are intended to support the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues and needs by implementing targeted interventions designed to provide timely, high-quality, and efficient care. CCBHC Targeted Case Management services are organized around goals aimed at providing access to services that encourage individuals to resolve problems that interfere with their attainment or maintenance of independence or self-sufficiency and maintain themselves in the community.
Essential Functions
The targeted case manager will refer to community and social support services, provide support for improved community service linkages, transitional care including appropriate follow-up for inpatient to other settings, support in navigation of complex healthcare and social services system provide individual and family supports advocacy for needed services performing own side crisis intervention when needed monitor follow up may include contact with service recipients the recipients families and all collaterals.
Promote community membership, development, and maintenance of natural supports.
Complete documentation using the electronic medical record.
Participate in staff meetings, in-service training, and supervision.
Work cooperatively with members of the team, adjunct providers, and other agencies to ensure timely and accurate service provision.
What are we looking for?
Registered Nurse (RN) or LPN (License Practical Nurse) required
Bilingual (Spanish/English) preferred.
Experience in human services, social work, and/or public health preferred.
At least one year experience in providing direct services to mental health and/or substance use disorder consumers.
Experience working for a community health center or Federally Qualified Heath Center preferred.
