The Case Manager in the Maternal Wrap Around (MWRAP) Program is responsible for working with women and their families to support and strengthen their capacity to engage in health practices to maintain stable homes through a family-centered approach. The Case Manager conducts needs assessments and provides intensive case management, wraparound services, and recovery support for pregnant/postpartum women with substance use disorders.

Essential Functions:

  • The Case Manager in the M-WRAP maintains a caseload of 30 families, focusing on developing a single, coordinated care plan for pregnant/postpartum women, their infants, and families in accordance with program guidelines. * Conducts home visits at least twice. * Advocates on behalf of the family. * Conducts assessments that include life domains. * Provides and documents case management services in compliance with program guidelines. * Maintains ongoing relationship with community and health care providers as well as other case managers, making referrals for program services. * Conducts networking with community stakeholders and recruits’ community participation in collaborative problem solving around barriers to care and in linking clients to services. * Completes all required reports and other documentation in a timely manner. * Attends and participates in supervisory, team, multidisciplinary, and other meetings in compliance with program guidelines or as assigned. * Maintains client and staff confidentiality in compliance with operational policies and procedures. * Provides services in a culturally competent manner.

Requirements

  • Bachelor’s degree in health, psychology, counseling, social work, education, or other behavioral health profession

  • 3 years’ experience working with high need families involved with substance use and mental health disorders

  • Training/education in trauma informed services

  • Evidence of working with substance use and co-occurring mental health disorder populations, or evidence of addiction coursework

  • Basic computer skills

  • Valid driver’s license, current insurance policy, and car

  • Knowledge of community resources that provide services for women and their families (i.e., early childhood intervention, child welfare)

  • Good organizational skills: ability to prioritize work requirements

  • Good facilitation and conflict resolutions skills

  • Good oral and written communications skills

  • Ability to work well with consumers and providers

  • Experience with maternal and child health issues and target population preferred

  • Bilingual English/Spanish preferred

Physical Requirements:

  • Must be able to see to read documents and hear well enough to communicate with others

  • Must be able to write, type on a computer, and use a telephone

  • Must be able to operate a motor vehicle

  • Must be able to lift up to 20 pounds

  • Additional requirements to be determined with input from Director and HR

Work Environment:
