Job Summary:

The Care Coordinator provides comprehensive, integrated planning, facilitation, documentation and reporting of collaborative and person-centered care, in coordination with public and private service providers. While ensuring correct data collection and reporting of each client's progress through the system, this job will internally facilitate client referrals in compliance with Coordinated Entry System standards and procedures. For the Continuum of Care, the Care Coordinator also organizes referrals for temporary housing, quick rehousing, and permanent supportive housing (PSH).

Minimum Qualifications:

Associate degree and Relevant Experience in Health or Human Services and knowledge and experience with recovery supports, community resources, housing, employment, and other professional and nonprofessional services. Experience working with those with substance use disorders mental health diagnosis, and chronic conditions is preferred.

Job Requirements:

Duties:

  1. Responsible for, but not limited to comprehensive assessment, outreach and engagement, service and treatment linkages and coordination using evidence based practices and outcomes.

  2. Comprehensive client centered Care planning development and management with the individual to assist in the integration of chemical dependency, mental health and medical services.

  3. Coordination of care with Finger Lakes Community Health Providers and FLACRA clinic providers as well as ongoing coordination of care with other providers identified in the care plan.

  4. Assessment of psychosocial supports and linkages with housing, employment, transportation, medical, dental and mental health care as well as community resources and care planning.

  5. Build and use effective communications strategies among peers, medical staff, addiction and mental health providers and other community agencies through the use of electronic assisted devices including Telehealth and other interactive technology.

  6. Help improve, measure, monitor, and sustain quality outcomes that focus on clinical indicators/performance measures, patient satisfaction, and plan adherence.

  7. Facilitate support of clients by working with providers for use of services, and expediting service

  8. delivery through linkage activities, shelter referrals and other housing intervention programs.

  9. Establishes and maintains collaborative working relationship with community organizations and

Resources.

  1. Work with provider staff to promote a shared knowledge and understanding of available community resources as clients are connected to housing.

  2. Ensure that all paperwork related to client and program activities is properly completed, managed confidentially, and stored in an appropriate location and submitted in a timely manner.

  3. Attend weekly staff meetings and case consultations as needed.

  4. Performs other related duties as assigned.
