POSITION SUMMARY:

The IDD (Intellectual and Developmental Disabilities) Services Facilitator works with children and families enrolled in program serving intellectually and developmentally disabled children ages 0-17. The IDD Services Facilitator is identified as the chairperson of the multi-disciplinary team and the developer of the Person-Centered Plan. The IDD Services Facilitator is responsible for assessment, referrals, linking and coordination of all services, as well as for monitoring and supervising prescribed services. The IDD Services Facilitator is also responsible for the clinical record.

 

EDUCATION AND / OR EXPERIENCE:

  • Bachelor’s degree in psychology, Social Work or related Human Services field required.

  • Must meet criteria for QIDP credential and maintain that credential.

  • Experience in working with and assessing children according to their individual needs and developmental level.

  • A minimum of two (2) years’ experience working with developmentally delayed, mentally ill, and/or disabled children and their families required.

  • Experience working in the home and/or community setting preferred.

     

    DUTIES AND RESPONSIBILITIES:

  • Responsible for the coordination and collaboration of all supportive services regarding children’s services for intellectually and developmentally disabled children ages 0-17.  

  • Assesses the needs of the family and begins the process of person-centered planning. 

  • Provides all referrals, linking and coordination of services for families in the IDD services program area. 

  • Provide individual, group and family IDD services sessions to assigned clients. 

  • Maintain a caseload size and weekly client service expectations as determined by supervisor. 

  • Maintain all paperwork, clinical records, reports, case notes, and all other client documentation in the appropriate electronic record systems, meeting all agency, contractual or billing requirements and deadlines.  

  • Monitors and supervises prescribed services and all paperwork involved in the process of service delivery. 

  • Maintains responsibility for the clinical record.  

  • Participates in multidisciplinary teams and school advocacy to ensure appropriate service delivery. 

  • Collaborates with the CMH system and the school system to ensure all goals remain in compliance with Medicaid guidelines and are not duplicate in nature. 

  • Through direct contact and correspondence, ensure Starfish Family Services has a positive relationship with all referral sources and related agencies.

     

    COMPETENCIES AND ABILITIES:

  • Understanding of child development.

  • Experience with cross systems service delivery.

  • Basic assessment of skills of areas of functioning and life domains.

  • Ability to develop and implement an Individualized Plan of Service utilizing Person Centered Planning and coordinate with other professionals and referral sources to meet client goals.

  • Competency, or willingness to learn, a wide range of treatment modalities that are individualized and person centered.

  • Knowledge of community resources and provision of supports coordination services.

  • Demonstrated ability to set and meet program outcomes and objectives.

  • Ability to communicate effectively both orally and written.

  • Ability to plan and organize time effectively.

  • Ability to assist families in crisis.

  • Ability to present self in a professional manner to staff, community, other social service agencies and referral sources.

  • Ability to problem solve and make decisions, especially during crisis situations.

  • Must have a working knowledge of all SFS programs within the first 90 days (about 3 months) of employment.

  • Ability to manage multiple priorities, function independently and demonstrate good follow through.

  • Ability to work positively, collaboratively, and productively as a member of a team with colleagues, supervisors, and agency staff.

  • Must be computer literate and have some experience with electronic medical records and Microsoft Office.

  • Ability to meet client service hours expectation as assigned by program supervisor.

  • Must have a commitment to the welfare of the children and families the program serves and to the delivery of quality services.

     

    LICENSING AND OTHER REQUIREMENTS:

  • Valid driver’s license and / or reliable transportation.
