Family Resource Center of Northwest Ohio, Inc. Community Care Navigator in Lima, Ohio

Who are you? You are a caring professional who is dedicated to providing mental health and/or substance use services for your clients. You have a passion for making a difference in your local community by providing community support services for clients while in their natural environment (home, school, community, etc.). Under the guidance of the Clinical Supervisor, you will serve as client advocate, acting in their best interest at all times. Attend treatment plan development and review meetings and coordinate/monitor the appropriateness of all services. Provide assistance in crisis situations to stabilize client and maintain client in the least restrictive treatment setting. Use crisis assessment tools and coordinate crisis services based on results; coordinate and implement universal screenings within the school systems. Knowing what is availability within the community is crucial to the well being of your clients needs. You will conduct assessments of community needs and develop plans accordingly. Conduct informational sessions and training for other community navigators. Participate in activities or studies designed to monitor and evaluate client outcomes. Provide resources, and linkage to services for your clients by maintaining a relationship with other agencies or external service providers as needed. Working in a team based environment is important to you. While working with the treatment team, you will maintain a strengths-based approach as you work with your clients and families to assist with the retention of dignity and the development of positive qualities in the client. In this grant funded position, you will regularly use various computer apps including Microsoft Office, our HRIS, EHR and other online systems, with multiple logins often simultaneously. You will have the ability to obtain an Ohio driver's license, maintain auto insurance, earn CPR/First Aid certification, and be willing to submit to background check and fingerprinting. You have:

  • HSD/GED
  • Previous experience working directly with youths and/or families

Preferred:

  • 3+ years of previous relevant experience in the behavioral health field or related field
  • Associate's degree or higher in social services or closely related field
  • Knowledge of community resources
