POSITION SUMMARY

Monitors Clay, Platte, and Ray County residents who participate in treatment courts. Provides oversight of consumers, to include coordination with treatment team to ensure follow through with court recommendations. Act as a liaison between the consumer and the courts. Provision of crisis back up weekly.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential functions include, but are not limited to the following:

• Acts as the court monitor for consumers involved with the treatment courts. Directs contact with judges, attorneys, and treatment providers in order to assure consumers are following treatment recommendations.

• Completes initial mental health court screenings with consumers to determine eligibility for treatment courts.

• Conducts weekly or biweekly monitoring appointments with participants to ensure follow through with court recommendations.

• Completes progress notes in AVATAR within 72 hours of service provided which clearly reflect interventions provided and necessity of those interventions.

• As needed, develops person-centered treatment plans initially and annually, which are strengths based, reflecting consumer’s life goals and individualized choices.

• Problem solves independently issues related to housing and housing crisis. Finds community resources to assist with basic needs, as well as emergency situations. Advocates for consumers in regard to receiving proper health care, and is available to problem-solve matters of significance and refer consumers to community support as needed.

• Serves as a resource for treatment courts and is available to staff problematic consumer issues or situations which may arise.

• Maintains knowledge of agency technology programs and/or systems needed to do the job and uses them effectively.

• Maintains required training and certifications.

• Maintains required agency productivity standards.

• Attends and participates in weekly team meetings and monthly all staff meetings.

• Other duties and/or responsibilities as assigned by the supervisor.

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)

Education:

• Bachelor’s degree in an applicable field required.

• CPR/First Aid certification required.

Experience Required:

• 1-3 years of position related experience required.

Required Knowledge:

• Knowledge of trauma informed care practices.

• Demonstrated knowledge of treatment court systems.

• Demonstrated knowledge of community based resources.

Skills/Abilities:

• The ability to use basic business equipment.

• The ability to develop person-centered treatment plans

• The ability to handle difficult situations and problem solve independently.
