Job Summary:

In accordance with the organization’s mission, vision and values, the Alerts Coordinator is responsible for the tracking and dissemination of information entered into and obtained through data platforms such as SPARS, RHIO, PSYCKES, e-Intervention, UniteUs, etc for information that supports quality client care and coordination of services. Care Coordinator will additionally assure agency and program compliance and fidelity in strategic business goals and will be expected to provide reporting to multiple departments.

Minimum Qualifications:

Associates Degree in Health/Human Services with a t least one year of working experience preferred. Pursuit of QHP is encouraged. Valid NYS Drivers’ License. Experience and/or training in Behavioral Health/Client Care.

Job Requirements:

Duties:

  1. Obtain, upload and enter information on NOMS surveys to the SPARS system. This information is time-sensitive and needs to be tracked for compliance.

  2. Maintain tracking spreadsheet to assure compliance with NOMS survey collection at baseline, quarterly health indicators reassessments and reassessment interviews.

  3. Monitor data for reassessment and quarterly health indicators reassessment rates, including regular (weekly) audit of spreadsheet against data identified as due in the SPARS system.

  4. Monitor quality of data being input into system and alert supervisor(s) to quality issues.

  5. Manage incoming RHIO alerts, direct alerts to specific departments and track outcomes.

  6. Obtain, upload and attest RHIO and PSYCKES consent information for all admitting clients to 28 E. Main programs including residential services/ outpatient.

  7. Support and train new staff on how to use RHIO, PSYCKES, SPARS and other platforms.

  8. Maintenance of program specific spreadsheets and others as assigned

  9. Demonstrates teamwork and an ability to work in a team environment with coworkers and supervisors. Demonstrates the ability to be flexible and cooperative.

  10. Maintain positive and effective relationships with community groups, referral sources and service agencies.

  11. Participate in general, staff, in-service and supervisory meetings held by the Agency.

  12. Other duties as requested by management.
