Position: Clinic Supervisor - Trinity Health PACE of Montgomery County

Purpose:

Your role centers on overseeing the delivery of nursing services to participants in the PACE Organization. From community outreach to clinic care, you ensure top-quality service and seamless coordination with the primary care team.

What You'll Do:

  • Manage day-to-day clinic operations, ensuring participant needs are met through effective communication and coordination with all departments.

  • Conduct initial medical assessments, contribute to care plans, and monitor participant health status.

  • Supervise clinic staff, oversee medication administration, and maintain quality standards.

  • Collaborate on financial and budgetary matters, ensuring efficient utilization of resources.

  • Coordinate participant care with external service providers and oversee specialty clinic services.

  • Facilitate integration of new participants into the healthcare system and ensure compliance with regulations

Minimum Qualifications:

  • Active Registered Nurse license in the state.

  • Bachelor’s degree in nursing (BSN) and CPR certification.

  • Minimum three years' healthcare experience, with geriatrics focus.

  • Proficiency in Microsoft Office suite and electronic communication tools
