Nurse Practitioner – Visiting Nurse Home Care Clinical Admissions

Summary:

  • Reporting to the Senior Director of Home and Hospice Services, provides professional nursing care as assigned. Coordinates care planning with other disciplines and providers to ensure the highest level care is provided to the patients.

    Responsibilities:

  • Ensures patients have timely and accurate orders prior to delivering services. Reviews Provider’s referrals for PMX, medications, treatments and visit orders.

  • Completes comprehensive and interdisciplinary assessments including: OASIS per regulations. Development and ongoing evaluation of individual plan of care based on needs, interests, and abilities in conjunction with provider, interdisciplinary team, patient, caregiver and/or representative. May complete ongoing visits to patients based on patients’ progress towards goals. Provides strategies to achieve patient goals in the plan of care.

  • Effectively integrate educational, medical, social and psychological factors when evaluating progress towards attainment of goals and outcomes. Orients, instructs and may direct work activities of licensed practical nurses (LPN), licensed nursing assistants (LNA), and students as appropriate. Evidence of ongoing communication between the nurse, LPN, and LNA regarding patient status and progress must be documented in the patient medical record. ARNP is responsible for the development of patient LNA plan of care and supervision of LNA services per regulations.

  • Acts as a clinical resource to the clinical team. This may be accomplished through education, joint visits etc. Collaborates with the clinical managers and education department in the provision of professional development for the clinical team members.

  • Complies with medical, regulatory, educational and records management requirements timely (e.g. insurance authorization, consents, physician’s referral, documentation) and maintains patient confidentiality in accordance with HIPAA guidelines.

  • Communicates with other providers and other health care practitioners (as appropriate) involved in the patient’s plan of care to exchange, discuss, and evaluate patient information for planning, modifying, and coordinating an appropriate treatment plan and goals. Assess for patient, caregiver, and/or representative and family counseling needs and make appropriate referrals.

  • Provide patient and/or caregiver education and training including but not limited to: diagnosis, interventions, care delivery, community resources, medications, safe and therapeutic environment, coping strategies, and discharge, etc.

  • Documents assessment, interventions, medication reconciliation, and discharge planning in appropriate clinical notes and plan of care into the EMR Completes documentation same day as visit, per agency standard. Participates in the agency’s quality assessment process improvement (QAPI) program to improve health outcome, patient safety and quality of care.

  • Meets agency obligations for, holiday coverage, weekend coverage, and on call shifts (as applicable). Complies with Agency standards for productivity, patient and staff schedules, and attendance at meetings.

  • Enhances professional growth and development through participation in educational programs, current literature,community based and home health agency in services, meetings, workshops, and provides training to staff, agency, and community groups. Complies with professional licensure standards and code of ethics.

    Requirements:

  • One year NP direct patient care experience or NP with 3-5 years direct experience as a RN.

  • Accredited RN Program

  • RN license - NH or other Compact State

  • CPR certified or within 30 days of hire

  • Driver license - valid
