Position Specific Duties/Responsibilities:

  • Assume direct responsibility for referral/intake process which includes the development, implementation, evaluation, and analysis of procedures, outcomes and program trends. Maintain ongoing contact with referral sources to ensure effective communication and constant flow of referrals in order to maintain maximum program utilization.

  • Work collaboratively with program directors and other staff to maintain an ongoing awareness of current and emerging placement resources and client needs. Respond to inter/intra-agency referrals and facilitate transfers as appropriate and needed.

  • Secure necessary information and documentation required to complete the intake section of the case record in a timely manner.

  • Identify and assist in the implementation of treatment plans for the foster, therapeutic foster and adoptive placements including establishing medical necessity at intake. Supervise the Permanency Planners in the implementation and oversight of the identified treatment plan.

  • Provide specialized Trauma Therapy with special cases requiring significant support. Interpret the family dynamics and treatment needs to the treatment team. Conduct thorough assessment of client needs and strengths.

  • Based on assessment of family dynamics and in collaboration with the Permanency Planner, ensure the most appropriate treatment plan is created and implemented. Assist in overseeing the provision of the services scripted in the treatment plan.

  • Through direct supervision of the Permanency Planners, help staff in identifying psychiatric supports, treatment, integration into the community, family case planning, behavioral techniques/compliance, and identifying evidence based interventions to best fit the identified youth.

  • Provide effective communication through regular participation in treatment planning conferences and on-going consultation with other team members. Assist as needed in scheduling and participating in initial meetings, Service Plan Reviews (SPRs), and Gateway Reviews.

  • Ensure effective strength-based permanency planning and development of therapeutic permanency planning goals.

  • Actively participate in professional and community organizations which provide ongoing knowledge of current trends and needs in the treatment of children and families.

  • Participate in the pre-certification and ongoing of training of Resource Parents (MAPP) as well as the home study process.

  • Accurate and timely completion and finding of all necessary documentation including but not limited to progress notes, re-certifications, discharge reviews, quarterly reports, billing, referral logs, etc.

  • Participate in the collaboration of the Solution Focused Trauma Informed Care Principles, which would include all training opportunities, group consultations, and the use of the core principles of collaboration, empowerment, safety, choice and trustworthiness.

Knowledge, Skills, and Abilities

  • Must have excellent communication skills

  • Ability to multi-task and be detail oriented

  • Ability to work independently and be an enthusiastic team player

  • Demonstrated knowledge of clinical issues of emotionally disturbed children and their families

  • Possess assessment skills, ability to engage families, and establish and maintain relationships with social services and other service providers

  • Specialized training in trauma treatment preferred

  • Ability to work effectively with clients, staff, families, and community contacts

Qualifications

  • Licensed Social Worker or Counselor with 2 years of related experience , foster care experience preferred

  • Valid New York State driver’s license and adequate auto insurance.
