In order to provide patient navigation services that focus on cancer screening and prevention to MCPN patients system-wide, the Patient Navigator will identify needs (largely based on health care team referral), resource identification, and support to MCPN patients to access referral sources. This position is a part of a 3 year grant cycle.
Provide linkage to resources (external, internal and community) as well as information for patients.
Act as a point person for care coordination services at MCPN.
Assist patients in navigating the MCPN system including Outreach & Enrollment, medical appointments and referrals, and internal resources.
Bridges the gap between clinician and patient by assessing needs and obstacles, following up with patients, and addressing health literacy, cultural issues, and social-class barriers.
Create and maintain relationships with MCPN staff and programs to facilitate quality patient navigation.
Assists program manager with reports, projects, etc. as needed.
Develops and maintains list of community resources as well as details on community programs.
Conduct chart review to acquire knowledge about patient status as well as past encounters with other MCPN staff and programs.
Provides timely documentation and required reporting for all services provided
Communicates and educates patients using native language, either in person or via interpretation services.
Performs duties in accordance with established clinic policies and procedures, objectives, quality improvement activities, safety, and environmental and infection control standards.
Assist in aligning and improving care connection between clinical and community settings for Medicare and Medicaid patients
Screen for Health Related Social Needs and connecting patients with identified needs to Community Based Organizations (CBOs).
Follow up with the CBOs to ensure the patient followed through with the Community referral and actually received the necessary service.
Focus on targeting and providing resources to patients who have Social Determinants of Health (SDoH) needs to effectively gather the necessary data to risk stratify and empanel to the appropriate care team.
Traveling to various MCPN clinic locations will be required to meet the needs of the grant.
Job Qualifications Knowledge, skill and ability:
Ability to interact positively and build rapport with patients, coworkers and/or external contacts.
Ability to demonstrate the capability to effectively and sensitively respond to the needs and concerns of the full range of MCPN’s diverse patient population.
Ability to handle sensitive information ethically and responsibly.
Ability to protect the confidentiality of patient, employee and business information.
A general working knowledge in the areas of patient navigation and health and wellness.
Strong interpersonal and problem solving skills.
Must be able to work independently with minimal supervision.
Education or Formal Training:
High school diploma or equivalent (GED) required.
4 years of college with emphasis on human services preferred.
Two years in a health care setting preferred.
Bi-lingual (Spanish/English) preferred.
Experience with diverse communities preferred.
A general working knowledge in the areas of patient care, health and wellness.
