In this position, you will be utilizing the Med Measure reports to see which Members' care is outstanding with specific Providers assigned to you.
Once these members are identified, you will reach out to them to close the gaps in care.
You will also reach out to the Providers to see what the next steps are in the process with the members.
You will be sending out Educational materials to the members, as well.
You will also mail them to remind them of services needed.
You will be responsible for educating the member on their Benefits and coverage during your calls.
You will be assigned a group of Providers.
Other duties, as assigned. Required Qualifications: High School Diploma / GED or higher 1+ years of experience providing Telephonic Consulting or Counseling in the Medical field. Ability to maintain confidentiality and adhere to HIPAA requirements. Experience with Microsoft Word and Outlook, with the ability to navigate within a Windows environment. Ability to travel up to approximately 25% of the time within the local area to Provider offices. Ability to work collaboratively with physicians and their staff Experience working in Physician's office, or Medical Clinical setting Preferred Qualifications: Experience in a managed care setting Knowledge of claims HEDIS experience.
