Responsibilities

Position Summary:

In keeping with the Pacific Central Coast Health Centers philosophy, the Licensed Clinical Social Worker provides Social Service activities through collaboration with the health care team, coordinating, and facilitating available resources to assist the patient through the continuum of care.

Qualifications

Minimum Qualifications:

  • Minimum of 3 years experience in an acute hospital or ambulatory outpatient setting.

  • Evidence of skills in communications (both written & oral), organization, creativity, leadership & group facilitation.

  • Expertise in one of the following areas: a. Assessment/counseling for adjustment to health problems, disabilities or loss b. Treatment of anxiety/stress reactions c. Behavior of cognitive problems d. Chemical abuse/dependence e. Complex discharge planning f. Child abuse/neglect g. Adult abuse/neglect h. Treatment of depression i. Suicide assessment j. Financial assistance k. Grief intervention l. Counseling for victims of violent crimes m. Sudden trauma or death. Ability to relate and maintain a rapport with all levels of professional personnel.

  • Currently licensed by the State of California as a Social Worker.

Preferred Qualifications:

  • Experience in treatment of depression, anxiety and eating disorders

  • Computer experience and knowledge in the management of computer data desirable.

  • Experience with performing bariatric surgery evaluations.

  • Bilingual

  • BLS Certification
