Provides direct nursing care to patients of the assigned clinic/practice in accordance with established policies, procedures and protocols of the healthcare organization.
Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's continued care needs with members of the healthcare team; will work in coordination with Family Nurse Practitioner and Medical Assistant.
Travel to multiple sites is required.
Hours are as follows:
Monday and Wednesday: 7 am to 6 pmTuesday: 8 am to 4 pm Thursday: OffFriday: 7 am to 5 pm
BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date. Advanced Cardiac Life Support (ACLS) & Pediatric Emergency Assessment, Recognition, OCN certification, and Stabilization (PEARS) preferred.
Licensed Registered Nurse credentialed from the Wisconsin Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date.
Graduate of a board-approved Registered Nurse Program required Bachelor of Science in Nursing (BSN) preferred
- Two years of Nursing experience preferred; preferably in a Med/Surg, Peds, ER or Clinic setting.
